Commissioning a Serious Case Review

If you feel we may be able to help, please contact Jane Wonnacott (In-Trac Director-professional practice) for an initial discussion. At this stage we will need to know:

• timescale
• the size of the task. i.e. for IMR's number of staff to be interviewed, location and amount of written material and for Overview Reports the number of agencies involved, the plan for involving family members, likely number of meetings to be attended by the author, who will be compiling the integrated chronology and whether a presentation will be required to the LSCB.

Should you wish to go ahead we will:

• Confirm the lead associate
• Agree a maximum number of days and a daily rate
• Confirm the timescale and review process
